Alert volume not matching ringer volume

I've got this weird thing going on where my alert volume is set to vibrate even though my ringer volume is at max. I've looked at the settings and it is set for the alerts to match my ringer.

When I tap my audiomanager widget I see both sliders (ringer/alerts) at max but the alerts slider looks dimmed out compared to the others.

Any idea what is goin on?
