X and signal loss?

My wife is having some issues with signal loss (red circle with line through it) periodically, and has to reboot to fix it, shes done the *228 thing, just wondering if this is a known issue or if something is wrong with her phone? She is still stock. Just wondering if there was a stock base band update that was missed?
