Weird charges on bill

Hey guys, I just glanced at my bill and didn't know what to make of it. Now, before you all say it's not that much difference, I know that..but I still want to know why it's different. With everything I know I signed up for: 59.99 plan, 29.99 data plan, 7.99 insurance, and with all the taxes, I get around $105 total, not $111. What's the other insurance listing for? I've attached a picture.

my proated bill is bs, my monthly bill is 200 dollars a month, they charged me 4 days prorated and it comes out to 42 dollars pro rated for four freaken days....arggg
