If you are talking about the one that has all different volumes i think its audio manager widget...there is a free version and a paid version

My current layout on my D-Inc. I'm using Launcher Pro Plus. I opted for scrolling docks in lieu of scrolling screens; I keep my main screen stagnant. Very happy with my cute little layout for now .
