ringtone goes missing

I have an mp3 that I use as a ringtone. Occasionally, the phone will default to a random ringtone.

When I go to check what tone is selected, nothing is selected.

I open the music app, set the mp3 to ringtone and it shows up in the list again and is selected.

I don't have any ringtone apps installed.

My phone is not rooted.

Any ideas would be appreciated.

I have noticed this is mostly due to usb plug in and unplug. Make sure you hit the eject on your pc before you unplugging the phone . just in my experience in this is what I have found

I am pretty scrupulous about safely removing hardware.
