Voicemail notifications still not going through!! please HELP!!

ok, so I called my Voicemail, and set notifications on, I also turned the delivery rate to "rapid" rather than "standard" but Im still not getting Voicemail notifications?? I was thinking that maybe I have an app that's preventing proper delivery and notification of new voicemails? any thoughts or ideas? thank you.

I can tell by the amount of replies that this is a pretty rare problem....

I doubt it's an app, I mean if you rooted maybe that did something. I would sbf and see if that does it.
