What is the best hands free walkaround kit?

I'm looking for a REALLY good microphone with a 1 ear canal cord. Has anyone had any experience with a really good one?

The ones I use people have a very hard time hearing me. I can't find a really good one. Any recommendations?

Just to clarify, I'm looking for this so that I don't have to walk around the city with the phone up to my face, I can have it in my right ear.
