My Note 4 keeps resetting my default ringtone

I've set my custom ringtone for when I get phone calls. It's set to an MP3 I've loaded on not only the phone but also the SD card (Settings --> Ringtones and Notifications --> Ringtones). But inevitably my custom setting gets undone somehow and the default phone ringtone is set back to "VZ Airways" (I'm with Verizon).

WHY!??!?!?!?! This is soooo annoying. I've used the same custom ringtone for years. When the stupid VZ Airways plays I don't recognize it as being my phone.

I think my setting for "Notifications" also gets re-set.

Anyone know what's going on?
