massive battey drain

here is my problem: just started, plug charger in says it is 40% charged when it is off and charging. turn it on while it is still plugged in[ac] 100%. as soon as i unplug charger, it turn off and will not turn on. when i plug back in and turn on, it say 5% battery and cuts off. did search on battery drain but did not get any answers. anyway, HELP PLEASE! thanks. i got this phone back from moto in may, brand new, have it rooted with sbf.
