I need to unroot my Incredible

I had my Incredible rooted about a month ago and I love it, but I accidently erased my original HTC rom and cant get it back. Also, with rooting, my voice nav doesn't work and my phone is randomly shutting down. I would like to put it back to it's original form and then try rooting it again to see if that helps. I have watched the "famous" youtube video but its not working for my phone. I may be doing something wrong so I really need step by step instructions.
