How to go back to "stock"?

I rooted my phone and tried flashing it several months ago, but was unable to unlock the booterloader or something... (it's been a while) Anyways, the OTA 9.18.94 update came out, and I wanted to go ahead and update my phone, but the update fails. I tried getting rid of the "root" and I did a factory reset on my phone. It now looks, feels, and smells just like stock, but that update still fails. Also, we just implemented a new MDM solution at work, and even it says that "my phone may be rooted". Again, this is after doing a factory reset... I've also tried using a system file explorer and manually deleting any SU files. Still, the update fails...

So, it's looking like I may need to flash back to stock, if that is possible, or if someone has something else I can try to "unroot", I am open to suggestion. I am currently running 9.18.79.XT926, if that tells anyone anything...

Can someone help?

update:

I was able to get the MDM software to finally stop detecting my device as being rooted. I found a couple more su files on the device and, after removing those and rebooting, the software no longer says my device is rooted...
the update still fails however.

well, i download the update, I go to install, the phone reboots
I see the android with the spinning multi-sided shape spinning in its belly... (god, I hope that makes sense)
and there is a progress bar beneath it. the bar goes to almost half way, then it shows the android fallen over with the exclamation point.
the phone boots up, and it says "the software update failed"

If you had cwm recovery installed, it would've told you which file gives error.

Have you tried installing supersu, then going to settings and clicking the full unroot button. Make sure you remove any custom recovery first but full unroot always worked for me

I installed ROM manager... it complains that my phone is not rooted. I dont think I can install the recovery piece, since I am not rooted any more. Is there a way I can manually try to install that update and see exactly what the error is? (without having to re-root my phone)

again, I dont think my phone is rooted any more, but just for kicks and giggles, I installed this prog, hit the full unroot button, which essentially just un-installed the program. then i rebooted, and tried that update again. It still failed.

I need a way to see exactly why that update is failing. preferably without having to re-root the phone, if possible.

another note: everything I have read so far says that when you download an OTA update, it is placed in the "cache" folder. I am using "ES File Explorer" and when I browse to /cache/, the folder is empty.
Any ideas where the update is downloading to?

apparently, I could not see inside of those directories because I was not rooted. I went ahead, bit the bullet, and re-rooted my phone. Then I checked the last_log file to see why the update was not working, here is what I found:
