End Call Takes Phone Out of Vibrate

Hello. I have just recently had an issue where ending a call, or just at random times, my phone goes from vibrate to ringer on. I've tried a few different switch widgets for Ringer/Vibrate/Silent and it still does it. Any idea how to fix?

For now I have a quick fix by downloading this Volume Locker program. So I have it enabled when I'm on vibrate so it doesn't just randomly go to ringer. Just wondering what has caused this issue.
