Diplay is not turning off and automatically locking

My mom's galaxy s4 does not automatically turn off the display and lock itself after a couple minutes.

I have the screen timeout at 30 seconds but is there another setting that i need to change?

Is there a possible app that she installed that's not letting the screen turn itself off? I'm worried about the pixels burning out or battery comsumption, etc.

There are several apps that would keep it awake but that doesn't really sound like the problem. Has she tried a power down and back up?

Also make sure that setting under Smart Stay is disabled.

Update: so it only happens while its charging. When i compare my dad's and my s4 with her's, ours will automatically time out
