Adding icons

The tap and hold screen to add items isn't working. Tap and hold gives the wallpaper option. What is my problem?

If your on ICS go to apps and long press the app and it will show up on the home screens.

I want to add a smart actions icon but I'm not able. My email and text notifier has stopped working and 'verizon wireless' is announced during an incoming call.
I would like to fix these issues.
