Screen banding/not working

I'm not sure if "banding" is the right word, but the icons/keyboard seem to digitally tear when moving (opening the keyboard or swiping screens). This is on a replacement phone and I don't remember seeing this effect on my original X. I have also done a few factory resets.

ALso, the touch screen seems to not react anymore unless I power off the screen and turn it back on. Looks like this one needs to go back.

Same thing happened to one of my replacement X's. If I scrolled any page up or down quickly, there was an invisible 1/4 inch band across the bottom half of the screen that would 'tear' any icons or text that passed through it. My next replacement was perfect.
