please help with signal lose

My phone has started acting strange in places where I always got 4g now I never do. When I make a phone call I lose all the signal I have done several hard resets but nothing work. I have not done an FDR and hope I don't have to thanks for any help and I am set on LTE/CDME.

Is your phone in a new/different case? Did you install any specialized communications apps about the time this started?
