phone boots into fastboot every time

I know this is something simple, but cannot figure it out. Whenever I turn on or reboot, it always goes into AP Fastboot Flash Mode (S) (Flash Failure) - 0A.61. How can I turn this off so that it doesn't come up all the time? TIA.
