Read through this entire thread but didn't see it mentioned...
3-way calling does not work on either of our company's DXs. After 3 levels of VZW support and another 2 levels at Moto, all they suggested was doing a factory reset (which didn't work).
Tried *228 on my own and still no joy. In reading posts on other sites, it appears this issue may be geo-centric (i.e. users in Denver can't use it unless they try it while in another geographic market).

I have the same problem. I'm in Fort Collins but I'm not sure if it's location related or not.
Funny enough (to me anyway), while VZ had me on hold while they were talking with Moto about it I kind of figured out a work around...
- Make your first call
- once that's established, tap the +Add Call button
- tap the green phone button at the bottom of the screen. This should give you a dial tone.
- dial the number of who you want to conference in (the dial tone will go away)
- again, press the green phone button at the bottom of the screen
- after a second or two you should hear the second call ringing and see a Merge Calls button. You can press this now or wait until the person answers.
There are some issues with...
- when dialing the second number you can't use your phonebook. You have to dial it by hand.
- when you press the green phone button and dial, the person on the first call can still hear you talking and dialing. Luckily, once you press the phone button again it seems to put the first call on hold while it calls the second person so the first shouldn't be able to hear the second person answer until you press merge calls. At least that's they way it's worked for me.
- if you make a mistake in dialing its a bit goofy to cancel the attempt. If you realize you made a mistake you can use the back button to edit the number before you tap the green phone the second time. But, if you already tapped the green phone the second time you have to wait for the Merge Calls button to come up and then tap it and then tap the End Last Call button that comes up until you hear nothing or the original call (it seems to cycle through a slow busy, a dial tone and then the original call). At this point you would press +Add Call again and start the process over
- If it's all worked up until now and you're done with your conference call you can drop both people by pressing the End Call button but you cannot select which call to drop. This seems to be a limitation of how the "app" is designed. You can, however, press the End Last Call button to drop the second call you made. This seems to work but the screen doesn't change. The top of the screen still indicates that you're in a conference call and the End Last Call button is still visible (It will still cycle through the slow busy, dial tone and first call but it won't get back to the second call, thank goodness)
Once you're completely done you can just press the End Call button to drop the whole mess.
