No Start after OTA update, rooted

Last night my wife allowed the over the air update from big red to her stock but rooted TB. Apparently it tried to restart and got stuck with the triangle and exclamation point android screen. I deleted the PG05img file from the sdcard after rooting, so it was not on there. I was able to walk her through a nandroid by accessing clockwork through hboot, life is good now restored to the saved nandroid but I'm waiting for the OTA update to try again. My question is has this happened to anyone else? Any ideas on why it didn't boot up with no PG file? I was expecting to just lose root access not have start up issues. Thanks in advance.

im fairly confident that if you are rooted you should not be accepting any OTA updates. it can unroot you or screw up the phone.
