issue unrooting droid incredible

i have tried 4 different ways of unrooting my droid.. i added the file to the sd card and rebooted into recovery but the file is not being picked up.. i need to return the phone for a warrenty replacment... please help!

Formatted to fat32?

I right clicked the drive for the sd card and it said fat32 under format. So im sure its correct. I also did a quick format and then added the file in question. Still no go. Once i reboot and go into recovery it has an update. Zip file that it automatically finds. No file that i loaded is there. Also it gives mea list of files that are not on the sd card. As if its coming from the phones memory

Also when i click and run that update. Zip the installation aborts.
