Here are two icon packs I created not too long ago.

Instructions:
Backup your ROM
Flash in recovery
Enjoy

Notes:
ICS versions are based on Scott's CleanICS framework
GB version is based on CleanROM GBE 1.6.5 framework
They may or may not work for other frameworks
Red ICS 4.0.3 is based on Xtreme Outcast's/Newtoroot's Senseless ICS framework.
