home screen forceclose message

Sometimes a foreclose notice comes up for the home screen app. I'm waiting for it to come up again so that I can tell you guys exactly what it says.
Why is this happening? I did clear data on the home screen app a few weeks back.
Also sometimes my toggle Widgets just disappear off my home screen.
Why is this happening.
