Galaxy s4 keeps freezing and restarting multiple times per day.

I had a S4 last week that randomly started freezing and restarting roughly 10-15 times a day. I took it in to Verizon and they did a hard reset and then decided to send me a new device. After getting it in and installing my SIM/SD cards, and doing all the updates, this phone has started doing the same thing as well. When the phone freezes it will be in the middle of an app opening, or while its asleep but will have a blue LED lit up at the top which will result in having to take the battery out.

But then when it restarts I will be in the middle of something or go to wake the phone up and it will vibrate quickly and restart. Then last night it started this thing of when I would try to open an app or function on the phone, it would prompt that there was an error and has stopped/failed. Also I keep getting an "unauthorized access to a secured area" prompt in the notification bar and when I go to try to update the policy, nothing really happens.

Where do I go from here? Its a brand new phone, no water damage or anything.
