Droid: SD Card from Droid X Mounting Issue

I recently purchased a Droid, and the previous owner said he replaced its SD card with his Droid X's card. Let's consider it a faulty SD card as a last resort.

The issue is that the SD card becomes unavailable over time or if I try connect it to my computer. The way I resolve the unavailable issue is by unmount/mounting in recovery (very annoying).

Does anyone have any ideas of a fix? Maybe there was something that needed to be done when switching a Droid X SD card to a Droid that was not. Thanks.
