Droid 4 not writting to file system

I'm trying to figure out why my Droid 4 will not write to the file system on it. All app installs get 919 unknown errors on it and fail. Any changes made to it are reverted upon rebooting it and to make matters worse flashing with RSD lite does nothing. Also tried factory restore and the phone is still unchanged. I went to copy a nandroid to restore the system to the internal sd card partition and hash checked it. It came back as bad. Even reformatted the internal storage part with windows and that did nothing. Still reverted back to the previous state. Any help would be great thanks in advance.
