Disconnected calls

I've had my Droid X for about 6 months. I have a problem with losing calls, sometimes 3 or 4 times with the same person. I'm guessing I'm holding wrong and I'm hanging up on people. Sometimes I put it on the dial pad but then I can hear the buttons being tapped as I'm talking. Is there a way to make the screen blank? Has anyone else had this problem? HELP!

The proximity sensor should turn the screen off when it's up to your ear. That said, it doesn't always do it.
