After not finding an option in the RAZR MAXX with ICS for formatting the SD and just seeing erase I started reading. Everything I read showed ICS took away the wording "Format" and used erase. Ok, with that information and knowing that issues arise with SD cards formatted outside - other phones or PC - I decided WTH. I formatted an SD card with my PC as NTFS -- unchecking quick format ----. After that format was complete I removed the SD card from the PC and placed it in the MAXX. A notification came up on the pull down stating that the SD card was blank or unformated. At that point an option was given for formatting and it does. Transferred all files back to the SD card and all is good.
I'm not sure if this will cure any issues for others but I now know the card is formatted for the phone.
