That looks great, I would like to know too.
It looks like Audio Manager. Its available in both free and paid versions in the market. Just note that the screen shot you have is of a black and white theme. Audio manager is in color.

Probably themed to go with the rest of that setup, which looks pretty smooth.

It's themed with 'Ghost Skin' on the market, .99... there are a lot of themes for it though the pro version says it has 80 skins and a theme kit or something, might get the pro if they have fixed it (it was buggy last time I tried it).
