Ring & Vibrate Switches to Ring Only Autonomously

Seems like since JB update, I can lay the phone down and later notice it has switched from Ring and Vibrate over to Ring Only. I keep it on Ring and Vibrate or Silent. I believe this is occurring without rebooting, but am not 100% sure. I am sue this is the result after rebooting the phone, though.

Has anybody had this happen and what is causing it? I have a feeling it is some app, but not sure how to determine that.
