my incredible keeps restarting

everytime i use my charger and take it out my phone just turns off and then turns on again.... one time it deleted all my scenes and i had no home pages should i try and get it replaced?

Perhaps flash a different ROM, see if issue persists.
Is the SD card acting up? Is SD card full?
