Did you format the card after putting it in the phone and before putting anything on it? Don't know if that is the cause but it's worth a try. Good luck!
This may seem trivial but make sure the card is pushed all the way in and is secure. Mine popped out shortly after insulation on my new phone. Weird thing is it worked periodically.
