No offense, Jeffery, but that does nothing to address the original question. By decypher, I was hoping someone could tell me what the information tells one.

Also, I have finally taken the S4 back to US Cellular. They replaced the SIM (yes, I had to re install all the apps and other information - 2.5 hours, so far) and I am hoping for the best.
