App permission?

Some apps ask for permission to "make calls that may cause charges"
Most of these apps are customizable apps like launchers. I was woundering exactly what this does and if its safe to install. Or will it run up my bill.

Home replacement apps have to ask for all permissions since they will have access to the whole phone. Since you can use it to acees the dialer it has to get that permission if I understand correctly.
