Intermittent can't mount sd card issue

When booting into sprecovery and trying to perform a backup or restore, I get an error "cannot mount sd card". Sometimes it works, sometimes it won't. When I get the error, I also can't see the card on my pc while using usb. Sometimes a battery pull works, other times not.
I am rooted.
It will always boot the phone normally although sometimes hangs when "preparing sd card"
Could this be a card problem, sprecovery problem, phone?
Any thoughts?

Just to clarify, I can always boot into sprecovery. I get the error when trying to perform a backup or restore.

Best Buy had PNY 16GB sd cards for 60.00. Thought I might as well give it a try. No more problems. I'll try formatting the old one and try it again too.
