Are you rooted or non rooted?
This use to happen to me when I was using ultimatedroid v7.8, as to why it happens I'm not exactly positive but I did upgrade to ud v10 and since then I have not had an issue

Sometimes when I go to my home screen there are no icons or widgets there, and the only way I've found to get them back is to restart, but sometimes they don't even reappear on restart and I have to restart again.
Any Ideas on how to fix this?

do they slowly reappear if you wait a while? like 10-20 seconds? i hav ehad this happen every now and then after switching between a number of different apps, i figure it just slow redrawing the screen because of the task switching, but don't really know.
anyways, if it is more than a 10-20 sec delay, i'd start by uninstalling apps you have downloaded one by one until the issue goes away - last app uninstalled is the likely offender.
if you haven't downloaded any apps and the screen doesn't redraw without a power cycle, then it's probably time to call your carrier.
Mine stayed long enough to make it into Verizon. They thought it might have been due to the wallpaper I was using, and the icons ended up behind it. This was a theory back in late Feb though so I don't know if that is still the thinking.
One time I replaced the icons and then a couple days later the original set came back, under the new set. I realized it when I couldn't read the text below the icons due to it being doubled up.
It can't hurt to bring it in or try a different wallpaper.
