At least once a month my Droid says damaged SD card and I have to format .
has this happened to anyone else,

bad SD card?
4 times this has happened

That doesn't happen to me. I've had this droid since release day in November.

I'd lay odds on a corrupted SD card. Might want to look into a replacement.
