I use the stock Verizon HotSpot app and it works perfectly, well with root and using the Entitlement Hack (Third-party apps do not work well with the hack.) Definetly have a look at Reverse Tethering, too.

Does the stock VZ hotspot app ask you for the acceptance of using their app still with the entitlement check?

With the Entitlement_Check, upon the initial set-up of running the Verizon Hot Spot app, there is a Security Warning. However, you can also check the box so that it does not appear again in future connections.
