System Software not authorized by Verizon Wireless

I have tried to install a ROM on a friends phone around 5x now and i keep getting the
"system software not authorized by verizon wireless has been found on your phone."
error message

which seems to work fine for root and i even see an open padlock on the boot screen which seems to me like an unlocked bootloader.
HOWEVER... after installing a ROM (tried a few 4.1.2 variants) i still get this verizon message.
luckily i am able to flash back to stock and start over without problems.
tried CWM and TWRP with no difference

im doing the rooting/unlocking AFTER the latest OTA... what am i doing wrong?

The yellow triangle is a sure sign that your boot loader is still locked. I myself just went through this last week. Had to recover/flash back to stock then start all over again.
