Currently using ADW ex. The dock text is custom via GIMP, and the Picture frame holding the icons was photoshopped onto the wallpaper before placing it on the phone.

What is GIMP? I've never heard of it

GIMP is a poor mans photoshop. It's a free download from the web. It does most of what photoshop is capable of, without the expense.
