I think I may have finally fixed this (though I don't really know the cause). If you have the issue, please try the following:

Download the latest ROM Manager off the Market: 1.8.4.0 currently.
    Start the application.
    Flash Legacy Recovery.
Choose 1.8.0.0. This is the recovery with the fix, it is a test recovery, so it isn't flashed by default.
    Try a backup
Then test a restore of that backup!

Please let me know if this fixes the missing md5 problem. Been trying to track down this bug for a while!

    ClkworkMod 1.8.0.0

Shut SetCPU at boot - unchecked

I noticed that if I try to rename the file when backing up through rom manager, it will not work when I go to restore it. If I leave the file name the way it is (full date and time), everything works fine.

Can you try 1.8.0.2? Any chance you can pop into IRC?

I just debugged this on wrongaddress's phone. It looks like the Camera app is somehow hosing the /cache directory on some phones and causing the backup to completely crash, before it generates the md5. If you clear cache, backups should work again.
