Looks like Dreamlyfe themes (Dreamlyfe Design: LPT). I've bought the original, lux, and sharp themes from him, and they all give a very clean minimal look to the homescreen.

It's Laulncher Pro Plus, with a dock from UltraLinx, and the text icons were just made in photoshop.
