Help with SD card

This weekend I hacked my Droid X and have the Liberty 2.0 ROM installed. This morning I received a SD card error message so I rebooted and now it is saying there is nothing on the SD card and asking if I want to format. Has anyone had this happen to them? I have tried reformatting via the phone and nothing happens.

Yep. Sorry but its dead. If its the original card that came with it call Motorola and they will send you a new one. That just happened to me two days ago.
