There isn't one for 4.5.621, it soft bricked a couple phones, you can use the MX_4.5.604 version though, it flashes the nTelos Milestone X 4.5.604...

Seeing the transfer mode switch just means that the phone saw the USB, not necessarily connected to the PC. You may possibly have a damaged USB...
Make sure you are in bootloader mode, make sure you are using the stock Motorola USB cable that came with the device, try different USB ports.
