Losing widgets

Stock ICS on current OTA ROM, rooted, Nova Launcher as default.

When using the app tray widgets tab in either the stock launcher or Nova, I "lose" widgets. Specifically, the 1x1 HD Widget and the Favorite Contacts widget (4x1). They both appear in the Widgets menu if I long-tap to add widgets in Nova, but the Favorite Contacts widget appears as a 1x1 widget and I have to resize it to 4x1 to get the proper appearance.
This does not occur immediately after factory reset, but an indeterminate time thereafter. There may be other "missing" widgets, but I don't know what they are because I don't go looking for them.

Anyone else seen this
