Can anyone identify where to get this notification bar?

Hmm...fairly standard smoked glass look...

It's actually Sense UI themed. Was able to find one with a white interior but not a black.

It looks like a desire/smoked glass combo. The bar itself is desire based but the interior is smoked glass.

if you have trouble finding one you can just do it yourself by swapping out the desires background with the smoked glass one in the framework-res.apk

That looks like my blackmod theme. There are a few versions floating around, which ROM are you using?
