Taking Requests for Icons/Headers

I will start taking requests for anyone who wants me to make them some custom headers and icons (for use with desktop visualizer).

I know there is a thread already for Win 7 themes and stuff, but in order to keep it organized, i'd like to have all the requests here.

Here are a few examples of what I can do:

Color of the Icon/Header:
Style (plain color/grungy/subtle/etc):
Size (1x4[tall];4x1[long]; 2x2; etc):
Name of the APP:
Syle of font used:
Picture (if you have one):
Special Directions:
