What would cause this to happen? I did the reset, but it's still there. Do I need to erase the phone's internal storage or something?

You have to unroot and uninstall the SU app. Otherwise it wont go away. The SU app installs as a system app that's why it wont go away with a FDR.

Yes, I'm currently on ICS. 4.0.4. Let me know and if you have the link, thanks.
What utility or process did you use to root your phone? Just curious because depending on how you rooted your phone, there may be some things you would do differently. I am familiar with Matt L Groff's utility 1.8 or 1.81.
Do you have voodoo root keeper installed? Use that to unroot then as jstafford mentions in the second post uninstall superuser from your phone's root directory.
If I'm not mistaken, Rootkeeper only saves a backup of Superuser, so if you "Temp Unroot" to take an OTA, I believe it saves SU somewhere else in the phone, and then restores it when you re-root. But it doesn't delete it.
I used Matt's utilities to root my phone. It doesn't get any easier.![]()
So MattyP how do you get all the ladies??
'MattyP' I just offer classes on upgrading their OS...
Really?? That's it??
'MattyP' yeah, the chicks love it when you give em an upgrade...

Oops, my bad... my instructions are in reverse. Superuser is installed in the root directory and can not be uninstalled through typical means (like an uninstall button). So he would have to be rooted then go into the root directory of the phone to delete the superuser apk and then unroot using voodoo root keeper.
I used Matt's utility 1.7 to root my phone.
