gymbook is your best bet
I use an Excel spreadsheet in Documents To Go. 1 worksheet for each exercise. Rows for dates, columns for weight and reps-by-set. Nice and easy.
I created the initial spreadsheet in Excel on my laptop and copied it to the sdcard rather than trying to type it all out on the droid.

Im trying to see if i can set up the same type of thing with the google docs spreadsheet.
I tried WorkItOut - seems like a good concept but i couldnt figure out how to display a list of the data after entering.
Havent decided if i want to pay for gymbook - wish there was a trial version.
After all of the clicking it takes to enter data- Im wondering if its just easier/faster to use the old fashioned pen and mini-notebook....
I used to use JEFIT. It has a free version and a paid version which is only 4.99. It has pretty much everything you can ever think of for keeping records and scheduals. It even has more exercises that I could even think of, as well as telling you which part of the body they work out..
I use an app called Strength. There's a free version with ads, and it explains how to do a bunch of different lifts and has videos of the right form. Best part is it's very easy to set up. It doesn't support every type of workout, though.
